The following forms must be printed to be completed.  See the details listed for the specific form you need. 

• Referral Questionnaire - The Referral Questionnaire is a PDF and can be filled in on-line but it CANNOT BE SUBMITTED on-line. You MUST print out the completed form and either fax or mail to the Local 131 office. 

• Change of Address - website ONLY!

• Welder Continuity Form - The Welder Continuity form MUST be printed and completed by hand and then mailed or faxed to the Local 131 office.
